Post-Hospitalization Care

Helping People Thrive Through Life’s Transitions

Post-Hospitalization CareReturning home from a hospital stay is both an exciting and precarious time. The days immediately following a hospital discharge are crucial for good care, as one of the leading causes of re-hospitalization is a lack of support during the hospital to home transition. At Midnight Sun, we understand that clients and their families are healthier when they are well supported through health transitions. Our post-hospitalization care services allow our clients to receive the care and assistance they need to properly transition from hospital to home and to allow time for proper healing.

Our Welcome Home service provides professionally trained caregivers who can assist you or your loved one in returning home from the hospital, same-day procedures, or doctor visits. Our experienced Anchorage caregivers ensure that you or your loved one is comfortable and well taken care of to reduce the risks of re-hospitalization.

Post-surgery care includes (but is not limited to):

  • Six hours of exceptional care
  • Transportation home
  • Medication assistance
